The role of the CHC Social Worker is based within the hospital discharge team at Whiston Hospital, for service users who are discharged from hospital into a nursing bed they are followed up by the completion of a CHC Checklist and DST were required.

The role comprises of the monitoring and tracking all those who have transferred from hospital and liaising with District Nursing Liaison Team to ensure that they are aware of the services users and the need to undertake the checklist.

You’ll be working within our Hospital Discharge Team

The Hospital Discharge Team are based on site at Whiston (STHK) and Aintree (LUFT) Hospitals.  They undertake social care needs assessments for patients on wards for whom they receive an Assessment Notification. They work with patients to identify suitable care and support arrangements to enable them to be discharged safely and effectively, with ‘Home First’ the guiding principle of all hospital discharge related activity.

Whilst the Hospital Discharge Team consists only of staff from Adult Social Care, the overall discharge process is managed by a team of health and social care staff located within the St Helen’s and Knowsley (Whiston base) hospital and Liverpool University Hospital Trust (Aintree Hospital) site. This multi-disciplinary team which works collaboratively with partner authorities such as Liverpool, Sefton, Halton, and St Helen’s Councils; the team also works with colleagues in housing, Police, environmental health, district nurses, therapy, matron’s, consultants, and GPs to ensure safe discharges from hospital are facilitated in a timely manner.
