Communities Team

People who are deemed to require further support from Adult Social Care following initial assessment and a period of support from the Adult Social Care Prevention Team, are referred to the Communities Team for ongoing / longer-term input.

The Communities Team supports Older People, adults with Learning and Physical Disabilities, adults with Mental Health issues who are not known or open to Secondary Mental Health Services, and people who require ongoing support to address substance misuse.

The team also fulfils Adult Social Care’s responsibilities under the Care Act to carry out transition assessments for a young person in order to help them plan and prepare for adulthood.  Young people who are supported by Children’s social care who will require ongoing support from adult services are allocated to the Communities Team.

The team also works closely with Merseycare’s Learning Disability Community Team and Autism Spectrum Condition Assessment and Diagnostic Service based in Whiston; the service is made up of a range of health care professionals including psychologists, mental health nurses, speech and language therapists and occupational therapists.
